Clinical research is critical to advancing surgical care globally. Yet, across both high-income and low and middle-income countries, systemic barriers continue to hinder progress. A recent global mapping review highlights the urgent need to address structural gaps in surgical research capacity if the 2030 goals set by the Lancet Commission on Global Surgery are to be met.
